Transition Care: Empowering young people transitioning from paediatric care to adult care
I am currently working with the Mater Hospital in Phibsborough to re-imagine how young people transition from children's care to adult care in the Congenital Heart Disease (CHD) Clinic. Currently I am working closely with Clinical Nurse Specialists and young people to gain insights into the frustrations and fears faced by young people during this transitional phase of their life.
Currently, I am establishing cultural probes and workshop methodologies that will be used to gain further insight into the role of medical staff and what it is like to live with a complex heart condition. Probes thus far include interactive waiting room probes, packaged workshops and transition care passports to help visualise care history for both the young person and medical staff.
The project began on the 19 May and will run until 01 September, 2025.
